Health Forms

Health Care and Health Insurance

Both domestic and international students need to have health insurance if they plan to live and study at Spring Arbor University. (This is not provided by Spring Arbor University, so you will need to select a health insurance service and provide us with verification of your coverage. If you need assistance in selecting a health care policy, we do have a few recommendations we can provide to you that some of our international students have used in the past. Let us know and we can give you a few options.)

Visa Information

Read the following information about obtaining your visa.

I-20 Form

Spring Arbor University can issue your I-20 Form, once we have received your first year’s payment and your health forms. The I-20 Form can be sent to you up to 90 days prior to our “program start date.” That date is the beginning of Welcome Week. Your original I-20 Form and official acceptance letter will be sent to you by express mail.

I-901 Form and and $200 remittance fee

Once you receive your I-20 Form, you will go online to FMJfee.com to complete the I-901 Form and pay the one-time $200 Remittance Fee required by the United States Department of Homeland Security. You must have your I-20 Form in hand to complete the I-901 Form. The required $200 will be paid to the U.S. government, not to Spring Arbor University.

F-1 Student Visa

Most international students are required to make an appointment with the United States Embassy in your home country to apply for your F-1 student visa.

Entering the United States to attend school

International students cannot enter the United States more than 30 days prior to the “program start date” that is indicated on the I-20 Form. This is a United States government regulation over which Spring Arbor University has no control.
